Consultant- Lead Consultant Bottle Neck Analysis Study

Background

In Tanzania children aged 6-59 months are supplemented by VAS twice annually during child health and nutrition months (CHNM) every June and December. During those specific events a comprehensive package of services beyond VAS is provided to children including immunization, provision of deworming tablets, growth monitoring, and nutrition counseling and education to caregivers. This service is mainly provided in health facilities and through outreach services specifically in hard-to-reach areas. VAS data are collected by health providers in the health management information system (HMIS) tools including tally sheets that feed into the national DHIS2 database.

Despite consistently high VAS coverage reported through administrative tally sheets, the persistent low-trends-survey-coverage data raised concerns on the reliability and validity of the tally sheet administrative coverage data among stakeholders, thus requiring a standardized methodology to assess coverage following distribution of Vitamin A. Further, evidence suggests that once the routine immunization services is completed by the age of nine months most caregivers do not take their children to a health facility to be supplemented with vitamin A. On the other hand, it is difficult to supplement children ages 36-59 months because they are enrolled/attending school as per the government education policy. All these arguments are in line with the Post Event Coverage Survey (PECS) that was jointly implemented by

Helen Keller Tanzania Office and Ministry of Health (MoH) completed in October 2022. Results of the assessment indicated all in all the three regions visited including Dar es Salaam, Mara and Kigoma had the coverage of less than 50%. To support improvements and ensure more children are covered, starting February 2023 Helen Keller International is supporting Mara Region to effectively implement CHNM through capacity building of healthcare providers, supporting micro planning sessions, supportive supervision, and distribution activities. On the other hand, we are also working with the government and nutrition stakeholders to explore ways to integrate VAS into the routine health and nutrition services. Given we are already supporting Mara region, we plan to conduct a two phased research studies that will support implementation of routine VAS.

Scope of Work
This term of reference focuses in seeking technical leadership to support Identifying bottlenecks in health service delivery that affect implementation of VAS.

Consultants will work closely with the Regional Secretariat and local government under the overall guidance and oversight from Helen Keller INTL through a consultative process and will be specifically responsible for the following:
1. Conduct detailed bottleneck analysis based on WHO analytical framework which disaggregates the health system into six core components also referred to as building blocks, leadership, and governance (stewardship), service delivery, health workforce, health information system, medical products, vaccines and technologies, and health system financing.
2. The consultant will liaise with all the district government and partners involved in VAS implementation to identify the bottlenecks in implementing Child Health and Nutrition Month (CHNM).
3. Draft the report identifying all bottle necks in the health systems. The report shall inform the resource needs, scope and government capacity to implement strategy, including Human Resources, financing, supplies, governance, information systems, M&E and capacity building.
4. Based on the findings, the draft report shall include recommendations on the scope of coverage, package content, targets as well as assumptions and risks.
5. Share findings with the Regional Health Management Team and Helen Keller INTL

Qualifications/Eligibility
· At least a Master’s Degree in Nutrition, Health System and Public health or a related field (PhD is an added advantage).
· At least 5 years of professional experience working on multi-faceted projects including activities related to the development of government policies, strategies, budgets, and/or plans
· Experience in carrying out similar studies in Tanzania/East Africa
· Excellent analytical, conceptual, communication, presentation, facilitation and writing skills.
· Strong experience coordinating activities with multiple stakeholders to optimize use of limited resources.
· Experience working effectively in partnership with collaborating organizations including government officials, international donors and agencies, and other relevant stakeholders.
· Strong knowledge of multi-sectoral nutrition is a plus
· Ability to respond rapidly to shifting implementation scenarios.
· Excellent English language skills
· This position is based in Dar es Salaam; he/she must be willing to be in Mara during data collection.
